Begin the Registration Process Here

Once you have completed the forms, please return them in person or by mail to the school office, via email to ngingras@richmondconsolidated.org or by fax to 413-698-3199.

 

STEP 1 - Fill out the required forms below:
 

All incoming students in Grades Prek-8 must complete the following forms:

STEP 2 - Send a copy of EACH of the required documents listed below:
 

  • Valid PARENT or LEGAL GUARDIAN Photo ID: State-issued ID, Driver's license or Passport 

  • Birth Certificate 

  • Immunizations 

  • Last physical exam record

  • Proof of Residence—such as a current utility bill or rental agreement (if unavailable then two documents with address listed such as bank statement, credit card bill, insurance bill)

  • Students with an IEP—a copy of the student’s Individual Education Plan

Until you receive a phone call or confirmation email from the school, your child is not actively registered. Registration is not complete until ALL forms and requested documents have been received by the office and you have been contacted by phone or email. 

Foster Students:
 

If it is in the best interest of a student in foster care to leave the school of origin, the student will be enrolled immediately. To minimize disruption of the student’s education, student will be enrolled without waiting to receive the documents listed above.
